Helping high-risk patients access life-changing health care with the support of donors, sponsors, and community partners.
What is Reach7 CARES?
Reach7 CARES is a third-party sponsored program that provides diabetes screenings and subsidized care to high-risk individuals in our local communities.
We partner with local charities, places of worship, senior centers, healthcare plans, employers, and individual donors to help our families and peers Live Better & Thrive Longer.

Access to quality care is becoming scarce, with provider wait times often between 6-12 months
Diabetes and obesity are chronic. When unmanaged, they result in heart disease, stroke, vision loss, kidney failure, limb amputations, neuropathy, and premature death.
These conditions cost our country over a trillion dollars a year in direct medical costs and lost productivity.
Through CARES, we can reduce or completely prevent serious health complications, costly hospitalizations, unnecessary ER visits, and absenteeism for our loved ones and peers.
Cost Savings per Intervention
Improving a patient's A1c level from >9% to <7% ("#reach7") saves an estimated $8,000+/year in total annual medical costs.
An A1c is a summary of a patient's blood glucose levels and diabetes progression. The higher it is, the more likely the patient will develop costly diabetes-related complications.
Many healthcare professionals and organizations, including the American Diabetes Association (ADA), recommend patients with diabetes reaching an A1c of 7% of lower.
