If you're living with diabetes and struggling to find or afford high-quality care, Reach7 may be able to help. Thanks to our generous donors and sponsors, our CARES program offers subsidized services for eligible patients.
Step 1.
Get Screened
You can schedule a free screening at any participating Reach7 location, or attend one of our sponsored outreach events, typically at a health fair, employer wellness day, church, or community center.
Once screened, a Reach7 team member will review your results and discuss next steps.
Step 2.
Apply for CARES Assistance
If your screening shows that you're at risk, your provider may recommend you for the CARES Program. All applications are reviewed by an independent committee based on:
Medical need and diagnosis
Financial eligibility
Motivation and willingness to participate
Availability of funded slots at your location
Reach7 CARES slots are limited, so you may be added to a waitlist until a slot frees up.
Step 3.
#reach7 together
If accepted, you will gain access to all the benefits that our Reach7 members receive, subject to any limitations of your donor or sponsor.
