The Reach7 Ambassador Program (“Program”) is offered by participating Reach7 Diabetes Studios locations (“Reach7,” “we,” “us,” or “our”).
Each Reach7 location is independently owned and operated. Participation, eligibility, and reward fulfillment may vary by location.
By applying to or participating in the Program, you (“Ambassador,” “you,” or “your”) agree to these Terms and Conditions.
1. Program Purpose
The Reach7 Ambassador Program was created to make it easier for people to take their first real step toward better health.
Ambassadors help raise awareness, inspire others to begin care, and connect their communities to personalized diabetes support.
Together, we can make prevention and care more accessible for everyone.
2. Eligibility
Individual Ambassadors
Must be a current Reach7 patient or member or an approved individual in good standing.
Must complete the official Ambassador Enrollment Form and receive written approval before referring.
May earn up to 3,000 dollars in rewards per calendar year, unless otherwise approved by Reach7 on a case-by-case basis.
Organization Ambassadors
Must be an approved business, nonprofit, or community organization in good standing.
Must complete the Organization Enrollment Form and provide a valid W-9 (EIN required).
Must receive written approval before referring.
There is no annual earnings cap for organizations.
Organizations cannot apply credits toward their own accounts, as they are not Reach7 patients.
Exclusions and Compliance Responsibility
Participation in the Reach7 Ambassador Program is subject to all applicable federal, state, and local laws and regulations.
It is the Ambassador’s sole responsibility to ensure that their participation and any rewards earned comply with all professional, organizational, or contractual obligations they may be bound by.
Reach7 is not responsible for determining or advising on an Ambassador’s legal or regulatory eligibility.
By enrolling, you represent that your participation in this Program does not violate any laws, professional ethics rules, employment agreements, or other obligations.
Participating Reach7 locations reserve the right to approve, deny, or terminate any Ambassador at any time.
3. Qualifying Referrals
A referral qualifies for a reward only if all of the following are true:
The referred person joins a participating Reach7 location as a brand-new member (not returning, transferring, or an additional member as part of an existing Household or Duo plan).
The referred person completes the intake form listing their Ambassador or Ambassador Organization by name.
The referred person is not enrolled through the Reach7 CARES Program or any other subsidized membership or program.
The Ambassador is approved, active, and in good standing at the time of referral.
The Ambassador submits a Referral Claim Form within 60 days of the new patient’s start date.
Only one Ambassador may claim a reward per new membership.
Patients who join through Reach7 CARES, promotional screenings, or any program funded by external donors or sponsorships are not eligible for referral rewards.
4. Referral Verification and Privacy
Reach7 complies with all HIPAA and privacy requirements.
We cannot confirm or deny whether a specific individual is a patient.
Referrals are verified internally through intake and claim data matching.
Valid referrals are processed within 30 days of verification.
All determinations of referral eligibility are final and made at the discretion of the participating Reach7 location.
4A. Privacy Waiver for Reach7 Patients Serving as Ambassadors
If you are a current Reach7 patient and choose to participate in the Reach7 Ambassador Program, you agree to a limited waiver of your HIPAA privacy rights for the following specific purpose:
You authorize Reach7 to disclose your name to new patients or members who identify you as their referring Ambassador.
This disclosure allows Reach7 to confirm that a valid referral occurred and that a Reach7 Ambassador properly referred the new patient.
No other health, treatment, billing, or personal information will be shared.
This waiver is limited strictly to your name only, and only for referral verification within the Ambassador Program.
By enrolling in the Reach7 Ambassador Program, you acknowledge and agree to this limited disclosure of your name for the sole purpose of referral verification.
If you do not wish to permit this, you should not enroll as an Ambassador.
5. Reward Options
Each verified referral earns a 250 dollar total reward value.
Ambassadors may choose one of three options:
Keep It
Receive a 250 dollar reward, issued as:
Individuals: gift card, or membership credit applied to the next full billing cycle or invoice after verification.
If multiple 250 dollar credits are earned in the same month, additional full credits will be applied to subsequent billing cycles in full 250 dollar increments.
Organizations: check, ACH payment, or gift card.
Share It
Split the 250 dollar reward between you and your referral.
Ambassador receives 125 dollars (via gift card or membership credit).
New patient receives a 125 dollar discount on their Reach7 membership.
Timing of Credit
All membership credits and discounts are applied to the next full billing cycle or invoice after verification.
Credits are applied in full 125 dollar increments only.
If multiple credits are earned in the same month and cannot all be applied to the next invoice, additional full credits will be applied to future billing cycles.
No partial credits or refunds will be issued for payments already processed.
The “Share It” option is available to both individual and organizational Ambassadors. Organizations cannot receive membership credits but may pass discounts to referred patients.
Gift It
Direct the full 250 dollar reward to be donated in your name to:
The Reach7 CARES Program, or
Another approved 501(c)(3) nonprofit or faith-based organization.
Donations are made by Reach7 and are not tax-deductible for the Ambassador.
6. Earnings Limits
Individual Ambassadors may earn up to 3,000 dollars per calendar year, unless increased on a case-by-case basis with written approval.
Organization Ambassadors have no earnings limit.
Earnings limits apply per calendar year and per participating Reach7 location.
Any increase must be approved in writing before additional rewards are earned.
7. Payment Process
Rewards are processed within 30 days after verification.
Payments may be issued as gift cards, membership credits, checks, ACH transfers, or donations, depending on the Ambassador type and reward choice.
Keep It and Share It membership credits are applied to the next full billing cycle or invoice after verification.
Rewards cannot be transferred, exchanged, or combined with other offers.
8. Taxes
Rewards may be considered taxable income under U.S. law.
Gift cards, checks, or ACH payments may be reported on IRS Form 1099-NEC or 1099-MISC if total rewards equal or exceed 600 dollars in a calendar year.
Membership credits are treated as price adjustments and are not taxable.
Donations through the Gift It option are not tax-deductible for the Ambassador.
Ambassadors are responsible for their own tax reporting and compliance.
A valid W-9 is required before total payments exceeding 600 dollars per year can be issued.
9. Prohibited Conduct
Ambassadors must not make false or misleading statements about Reach7 or its services, use spam or unauthorized advertising, submit fraudulent or ineligible referrals, disclose patient information, or seek compensation for medical referrals.
Violations may result in termination from the Program and forfeiture of any pending rewards.
10. Program Changes and Termination
Each participating Reach7 location may modify, suspend, or terminate this Program at any time without notice.
Reach7 may adjust reward amounts, eligibility criteria, or claim deadlines at its discretion.
Reach7 reserves the right to deny or revoke Ambassador status or rewards for any reason, including suspected abuse or non-compliance.
11. Privacy and Data Protection
All personal and referral data is handled according to Reach7’s Privacy Policy and applicable privacy laws, including HIPAA.
Ambassadors may not publicly identify or imply that any individual is a Reach7 patient.
12. Relationship of the Parties
Participation in this Program does not create an employment, agency, or partnership relationship.
Ambassadors act solely as independent participants and are not authorized to represent Reach7 in any professional capacity.
13. Limitation of Liability
Reach7 and its affiliates are not liable for delays in processing rewards or verifying referrals, technical errors, or indirect or consequential damages.
Total liability is limited to the total rewards earned by the Ambassador within the prior 12 months.
14. Governing Law
These Terms are governed by the laws of the state in which the participating Reach7 location operates.
Any disputes arising from this Program will be handled under those same laws.

Reach7 may modify, update, or replace these Terms and Conditions at any time without prior notice.

By continuing to participate in the Reach7 Ambassador Program after updates are posted, you agree to be bound by the latest version of these Terms and Conditions.
It is your responsibility to review this page periodically for any changes.
If you do not agree to the updated Terms, you must discontinue participation in the Program.
